The increase in "weekend warriors-" dentists who take weekend courses to learn dental implant procedures, has given rise to the creation of the Institute for Dental Implant Awareness, which hopes to oversee industry changes, protect patients and provide standards for privately-run courses on the topic.

COLLEGEVILLE, WILLIAMSPORT AND PHILADELPHIA, PA – The shrinking economy has affected virtually every walk of life, and the dentistry field has not been exempt.

More dentists are branching out by taking continuing education courses to provide dental implant services to patients in an effort to keep income levels steady.

The problem is that many dentists take as little as two days of training and then return to their office and provide dental implant procedures on their patients, said Williamsport, PA dental implants specialist Dr. David DiGiallorenzo, who has specialized in periodontics and dental implantology since 1995 and speaks internationally on the topics.

“Dentistry has eleven specialties, and dentists have a general scope of practice once they finish dental school,” said the Williamsport, PA periodontics expert, adding that specialists often train an extra three to five years after dental school, frequently earning an additional master’s or doctoral degree in their specialty in the process.

“There are no rules governing what a general dentist can and cannot do,” DiGiallorenzo said, “which differs from medical doctors, where a general practitioner wouldn’t dare do a heart procedure or an orthopedic procedure.”

But in the minds of many dentists, desperate times call for desperate measures, and many makers of dental implants seem to encourage dentists to branch out into implants.

“Unfortunately, the corporate side of biomedical devices in dentistry often sees the additional sales as a positive, even if is not in the profession’s or patients’ best interests,” said the Williamsport, PA periodontist.

Biomedical device manufacturers do not seem to overtly support the possibility that patient care is being sacrificed, he said, particularly after years of leading implant companies’ strategic plans focusing on training general dentists as their core growth opportunity.

As a means of addressing concerns surrounding this issue, the Institute for Dental Implant Awareness was formed in 1997. It is a non-profit corporation that provides independent, authoritative and objective information on the benefits of dental implants as an alternative treatment option for tooth replacement. The corporation also plays watchdog to these industry changes and works to protect the patient population and to provide standards for privately run courses supported by implant manufacturers, according to the institute’s website.

Note to Patients: Do Your Homework
The key for patients in need of dental implants to keep in mind is the importance of researching dental services providers before entrusting them with specialized treatments and procedures such as dental implants and periodontal surgery, said DiGiallorenzo, whose Collegeville and Williamsport, PA periodontics practices offer both services.

Although it is legal for any licensed dentist to place implants, not every dentist has had extensive training in implant procedures, nor do they have experience handling the potential complications that can occur, according to the IDIA website.

When performed by trained oral surgeons and periodontists, dental implant treatment is successful. These surgical specialists have undergone extensive training and experience with surgical procedures and complications, the site stated.

On the other hand, many “non-surgical” dentists who take a one- to three-day training course on implant surgery are not prepared to predict, diagnose and treat complications, should they arise.
It is important that dentists and patients alike understand that complications can occur, even when dental implant procedures are performed by experienced surgical specialists.

“The difference is that these specialists have a tremendous amount of training and experience in their corner,” DiGiallorenzo said. “They are better equipped to address complications, should they occur.”

Since implant placement is a surgical procedure, and there is no law that requires dentists to have extensive training prior to surgically placing implants, it is critical for patients to ask questions about training and experience prior to undergoing implant surgery.
The IDIA website has a page of information patients may use as a reference when selecting a dentist to perform their implant procedure.

Not every dentist has the experience and training necessary to place dental implants. Asking these questions of dentists will help you choose the most experienced, qualified professional for your procedure.

PHILADELPHIA, PA – Philadelphia PA dental implants expert Dr. David DiGiallorenzo has released a list of questions that should be asked of dentists when patients select a professional to place their implants.

It is important to select someone who has received extensive training in the specialty and who has a track record of success when it comes to performing the procedure, said DiGiallorenzo, a Philadelphia, PA periodontist.

“There are eleven specialties within the dentistry field, and dentists have a general scope of practice upon completing dental school,” said the Philadelphia, PA periodontics expert.

Those who wish to pursue a specialty typically must undergo an additional three to five years of training after dental school. Dentists often earn an additional master’s or doctoral degree in their specialty in the process.

Because there is a lack of rules that govern what dentists can do, many biomedical device manufacturers have begun offering weekend courses on dental implant placement and some dentists go back to their offices and perform these intricate procedures with just a couple days of training under their belts.

Following is a list of questions to help patients determine whether their dental professional possesses the training and experience necessary to place dental implants.

The Institute for Dental Implant Awareness recommends patients ask these questions if they are considering a general dentist, endodontist or prosthodontist to place their implants:

How long has the doctor been placing implants, and how many does she/he place annually?

How many surgical training courses did the doctor take, and how long was each course?

How many implants-related continuing education courses has the doctor attended annually?

How many courses has the doctor taken on managing complications related to dental implants?

Ask to see before/after photos that show the results that were achieved.

For dentists who are oral surgeons or periodontists, patients should ask these questions prior to selecting a professional, according to the IDIA.

How many years of experience does the doctor have in placing implants and how many procedures does he/she perform annually?

How long has the doctor worked with the doctor who will restore the implants, and how many cases have they done together?

Does the doctor have before/after photos to show the results that they have achieved as a team?

Studies show that there is a connection between periodontal disease and psychological factors including stress, anxiety and depression.

COLLEGEVILLE AND PHILADELPHIA, PA – Scientists and doctors who study the effects of stress on the body have determined that it can disrupt nearly every system in the body, and the mouth is not immune.

Published literature has identified a relationship between periodontal disease and stress, said Philadelphia, PA dental implants expert Dr. David DiGiallorenzo.

Fifty-seven percent of the studies included in a review published in a past issue of the Journal of Periodontology showed that there is a definite relationship between periodontal disease and depression, stress, loneliness and anxiety, according to the American Academy of Periodontology.

“Researchers continue to study the relationship between periodontal disease and stress, but most experts agree that patients who can minimize their stress by learning methods of better coping with it stand a good chance of reducing their risk of getting periodontal disease,” said DiGiallorenzo, a Philadelphia, PA periodontist.

In addition to increasing the risk of periodontal disease, chronic stress can raise blood pressure, increase the risk of stroke and heart attack, suppress the immune system, contribute to infertility, and make people age faster. Long-term stress can even rewire the brain, leaving people more vulnerable to depression and anxiety, the AAP article stated.

“There is a great deal of information out there that shows the stress hormone cortisol can have adverse effects on the immune system,” DiGiallorenzo said.

This connection is important when discussing periodontal disease because periodontal disease is a chronic infection mediated by the immune system and modified by risk factors including genetics, smoking, diabetes, medications and stress to name a few examples, said the Philadelphia, PA periodontics expert. Reflexology, exercise, yoga, meditation, acupuncture, a diet low in saturated fat and high in vegan components, and immune enhancement will help combat cortisol levels.

A study in the July 2007 issue of the Journal of Periodontology found that higher cortisol levels can lead to more destruction of the bone and gums due to periodontal diseases . Treating periodontal disease is important because if left untreated, it can lead to tooth and bone loss. As well, research has indicated a relationship between cardiovascular risk, diabetes and other systemic diseases effected by oral infections.

DiGiallorenzo offers the laser assisted new attachment procedure known as LANAP or laser periodontal treatment for treating gum disease. This is a no-cut, no-sew method to detoxify infections and stimulate repair of the gums and bone.

Stress Relief
DiGiallorenzo educates patients on the connection between stress and periodontal health, and helps them reduce their stress levels by offering a calm, comfortable dental spa setting that provides reiki, reflexology, massage, aromatherapy and color therapy to help patients relax and to create an optimum healing experience.

Reiki is a Japanese technique for relaxation and stress reduction that also promotes healing. It is performed by “laying on hands” and is based on the concept that there is an unseen life force energy that flows through people. People whose life force energy is low are more prone to feeling stressed out or getting sick.

Reflexology is performed by applying pressure to the hands and feet with deliberate hand, thumb and finger techniques without using lotion or oil. It is based on a system of zones and reflex areas that reflect an image of the body on the hands and feet.
Aromatherapy- the study of scents used in a therapeutic manner- is used in the dental spa setting for physical and emotional health and well-being.

Color therapy is a natural method of healing that taps into a person’s energetics and biochemistry to assist in regulation. It also can relieve stress by using a variety of colors through visualization, colored glass, colored oils or colored silk in a therapeutic setting.

“We have had numerous patients go into these types of therapies as skeptics and come out of them as believers,” DiGiallorenzo said. “There is a variety of ways to assist patients in stress relief and we are willing to employ whatever methods necessary to help our patients reduce stress and improve their periodontal health.”

Periodontal disease is second only to dry mouth in causing chronic bad breath. These tips can help keep bad breath at bay.

WILLIAMSPORT AND PHILADELPHIA, PENNSYLVANIA – Philadelphia, PA dental implants specialist Dr. David DiGiallorenzo recently released a list of tips for combatting bad breath.

The primary cause of bad breath is dry mouth resulting from medical conditions or medications, and gum disease is a close second, said the Philadelphia, PA periodontics specialist.

Bacteria that builds up in the mouth emits an odor, and the more bacteria that is present in the mouth, the stronger the odor can be, DiGiallorenzo said.

Following are steps people who battle chronic bad breath can take to help relieve the problem:

Get untreated cavities filled.

Make an appointment with your periodontist for a periodontal screening. Diagnosing gum disease is important because it is a silent killer.

If you smoke, quit. The tar, nicotine and other substances in tobacco smoke can build up on your gums, cheeks, teeth and tongue.

Chew parsley after meals. It contains chlorophyl, which is a natural breath freshener.

Chew sugarless gums that contain xylitol. It increases saliva production, which helps eliminate odor.

Have teeth professionally cleaned on a regular basis. For most, this means twice per year, said DiGiallorenzo, a Philadelphia, PA periodontst. For those who have periodontal disease, it could mean having professional cleanings done quarterly.

Use an oral irrigator (Waterpik) with an ounce of hydrogen peroxide mixed in the water to remove particles of food that get lodged between teeth.

When brushing after meals, remember to brush the gums and the tongue, as bacteria also can accumulate in these areas.

Brush well after eating sugary foods. Bacteria feed off the fuel provided by these foods.

Drink lots of water. It reduces the level of bacteria in the mouth.

Tobacco users are more likely to get periodontal disease and Dr. David DiGiallorenzo plans to use World No Tobacco Day to spread the word about this, as well as to inform the public about smoking cessation and periodontal disease treatment options.

WILLIAMSPORT AND PHILADELPHIA, PENNSYLVANIA – Williamsport Periodontist Dr. David DiGiallorenzo is using the World Health Organization’s World No Tobacco Day as a vehicle for educating the public on the dangers tobacco products pose to periodontal health.

World No Tobacco Day will be observed on May 31, according to the WHO website.
“It’s no secret that tobacco use can lead to significant health issues, but I don’t think that enough people have been educated on the fact that tobacco use can lead to some very serious oral health issues, specifically,” said DiGiallorenzo, who owns a Collegeville and Williamsport periodontics practice.

About half of all periodontitis cases are attributed to current or former smoking, according to a 2001 Journal of Dental Education article . The article also stated that cigarette and cigar smokers have “significantly greater loss of bone height” than nonsmokers. Pipe smokers also have been shown to suffer more bone loss than those who don’t smoke.

Those who use smokeless tobacco products aren’t any safer, DiGiallorenzo said. Smokeless tobacco users typically suffer from localized problems at the placement site, such as the cheeks of tobacco chewers and the lower front gum line and inside the lip in those who use smokeless tobacco referred to as “dip.”

“It’s important to me and the other Williamsport periodontists in the practice to educate people on the oral health dangers of tobacco products, but also to give tobacco users hope,” said the Williamsport dental implants expert. “Studies show that the progression of periodontal disease slows in patients who stop using tobacco products.”
To that end, DiGiallorenzo offers several smoking cessation modes within his practice, including nutrition, pharmaceuticals and hypnosis.

Using tobacco products can increase the chances of getting periodontal disease because it increases the likelihood of having calculus, which is plaque that hardens on the teeth and must be removed by having a professional cleaning, according to the American Academy of Periodontology. It also can lead to the development of deep pockets between the gums and teeth, and can cause bone and tissue loss, which provides less support to the teeth.
Untreated periodontal disease can cause pockets to deepen, which allows more bacteria into areas where it can destroy the supportive bone and soft tissue. Over time, the gums can recede from the teeth and the teeth may eventually become more painful, loose or could even fall out.

DiGiallorenzo treats periodontal disease virtually painlessly with the laser-assisted new attachment procedure commonly referred to as LANAP. It involves no cutting and suturing, he said.

Bad breath can be an important indicator of periodontal disease.

COLLEGEVILLE AND PHILADELPHIA, PA – Bad breath is something none of us want, but all of us get once in a while.

Bad breath can be caused by something as simple as eating garlic, or it can be an something serious, such as periodontal disease.

“The odor emitted from the mouth of someone with periodontal disease is pretty distinctive and most periodontists know the patient has gum issues even before they conduct the full exam,” said Dr. David DiGiallorenzo, a Collegeville, PA periodontist.

Periodontal disease rates second behind oral dryness in causing bad breath, said DiGiallorenzo, who also is a Williamsport and Collegeville, PA dental implants expert.

Between 85-90 percent of bad breath is caused by anaerobic bacteria that release sulfur compounds, which cause the bad odor, according to information on Carifree.com, a website dedicated to providing treatment solutions to the dental profession.

Because periodontal disease is most common in people age 35 and older, it becomes a bigger issue as we get older, according to http://www.animated-teeth.com/.

Periodontal disease effects the gums and bone that surround the teeth. The chief symptoms include tender and bleeding gums; missing, loose or shifting teeth; receding gums; and bad breath. As the disease worsens, it can damage the bone and can cause deep spaces to appear between the teeth and gums. These spaces are called pockets, and they are notorious for harboring bacteria, DiGiallorenzo said.

When these pockets get too deep, it is not possible to effectively clean them and that leads to bad breath.

Battling Bad Breath
DiGiallorenzo offers several suggestions to patients who visit his Williamsport, PA periodontics practice with questions about their chronic bad breath issues.

“A periodontal screening is the crucial first step to take to determine the ultimate cause of bad breath,” he said.

If it is a case of gum disease, Digiallorenzo often recommends utilizing LANAP- laser assisted new attachment procedure- a no-cut, no-sew, no-suture method of removing unhealthy bacteria in the pockets and repairing lost gum tissue and bone with the use of a laser light.

It’s also important to repair cavities because they provide a breeding ground for bacteria that lead to bad breath, he said Collegeville, PA periodentist Dr. Digiallorenzo.

Flossing and brushing the teeth and brushing the tongue regularly are imperative, DiGiallorenzo said. Two to four dental cleanings per year may also be required to ward off continued bacterial buildup.

Using an oral irrigator such as a Waterpik with a bit of hydrogen peroxide in the water also can improve oral hygiene.

Some dietary changes also can improve chronic bad breath issues, he said. Cabbage, Onions, garlic and curry can cause bad breath for up to 24 hours at a time, according to Health 911.
Canned tuna, spicy deli meats and certain cheeses also can cause bad breath. Sugary foods provide food for bacteria to feed off of, and tea and coffee drinks are thought to cause bad breath because of their acidity levels.

Healthy foods that don’t cause bad breath include papaya, pineapple, kiwi and leafy green vegetables.

Immediate load dental implants can provide teeth in one visit or secure a denture in one visit to restore form, function and aesthetics.

COLLEGEVILLE AND PHILADELPHIA, PA – Technological advancements are changing the way many dental procedures are performed, and one area where that is particularly evident is in dental implants.

Advances in nano technology, have facilitated bone cells to adhere to titanium surfaces faster, thus enabling Dr. David DiGiallorenzo to place healthy, beautiful teeth into the mouth in one visit.

DiGiallorenzo, a Collegeville, PA dental implants specialist, has performed these procedures for 15 years. A full mouth may take a half-day, while a single tooth replacement can be completed in less than two hours.

Immediate load implants are implants that can bear the forces of an anchor and a dental crown immediately after being placed into the jawbone with 97 percent long-term success, said DiGiallorenzo, whose PA periodontics practice has offices in Collegeville and Williamsport.

Historically, the original protocol was to wait six months prior to loading the teeth. This protocol was developed by the Swedish orthopedic researcher Dr. Per-Ingvar Branemark more than 25 years ago.

“What we found clinically and scientifically, is that if the forces where controlled properly and the implant installed with a different criteria, we could still achieve fusion of the bone and implants through a process called osseointegration,” DiGiallorenzo said.

One of the greatest benefits of dental implants is adding them to an existing denture patient, he said. Doing so enables the denture to be locked down, eliminating the need for sticky, smelly, messy adhesives and often removing much of the plastic on the upper roof of the mouth.

“Patients can really enjoy their foods, love life and have a lot more confidence with this procedure,” said the Collegeville, PA periodontist.
Age is not an issue when it comes to dental implants. DiGiallorenzo treats 80- and 90-year-old patients routinely.

“I remember a patient who hadn’t eaten a solid meal in years,” he recalled. “She is in her 80s now and she had two implants placed to secure her lower denture and ate a Philadelphia cheesesteak that night- something she hadn’t eaten in years.”

Immediate load implants also are a great option for fearful patients, DiGiallorenzo said. Any time fearful patients can reduce the number of trips they must make to the dentist, they are happy.

There are additional benefits. Patients also save money when the procedure can be completed in one visit. In instances where a patient must have a tooth extracted, DiGiallorenzo can reduce bone loss for the patient by immediately placing the implant after removing the tooth, he said.

Studies of immediate load implants show long-term favorable results, according to the Journal of Oral Implantology. Last year it published three case studies by a team of Turkish dental researchers showing that “immediate-load implants in healthy patients were successful and remained functional after 30 months of follow-up,”.
Not everyone is a candidate for immediate load dental implants. You must have good quality bone structure. However, if you have a thin jaw bone, Digiallorenzo will regenerate lost bone structure with the latest vampire technology where the patient’s own blood proteins are taken and processed to facilitate optimal healing.
“When we place a dental implant, we are shooting for optimum results,” he said. “That means that the patient needs to be in good general and oral health.”
If gum disease is present, it must be treated with the latest laser treatment called LANAP, which stands for Laser Assisted New Attachment Procedure. DiGiallorenzo described it as highly effective and added that it involves no cutting or suturing.
What happens when implants fail to integrate? Another implant of greater dimensions is placed with little consequence, Digiallorenzo said. There are some patients who just don’t tolerate titanium implants, due to metal sensitivities. This is about 4 percent of the population. For these patients, Digiallorenzo has been the first U.S. clinician to place the metal-free, zirconium dental implant.
“We have patients coming from all over the world and results have been fantastic,” he said. “It is aesthetic and highly biologic with long-term studies from Japan and Europe.”
For patients who are smokers, DiGiallorenzo offers several smoking cessation modes within his practice, including hypnosis, nutrition and pharmaceutical treatments.
“We want to create a good foundation before we perform the implant placement,” he said. “And we’re willing to do what it takes to get our patients healthy prior to performing the procedure so that they will be happy with the end result.”

There are five major signs of gum disease, and patients who exhibit these signs and symptoms should schedule a check-up with their periodontist to determine whether they need treatment.

PHILADELPHIA, PA – Gum disease is a serious condition that can indicate other serious ailments in the body and, without treatment, can lead to tooth loss.

Pennsylvania Periodontist Dr. David DiGiallorenzo has released his list of the top five signs of gum disease. People who exhibit these symptoms should consult their periodontist.

Gums that are swollen, red, tender and/or bleed easily.

This can be one of the earliest signs of gum disease, said DiGiallorenzo, a Pennsylvania dental implants and periodontal disease specialist.

Chronic bad breath or a bad taste in the mouth.

Foods such as onions and garlic are accused of causing bad breath, but there also can be far more serious causes, said the Pennsylvania periodontics provider. In fact, gum disease is the No. 2 cause of bad breath. Gum disease can cause deep spaces between the gums and teeth to form, which provide a space for bacteria to live in. Often, waste products from this bacteria cause the bad breath, according to Animated-teeth.com.

The teeth appear to be getting longer.

People with gum disease often begin to exhibit signs that their teeth are lengthening because the gums actually are receding, leaving more of the teeth exposed.

Teeth feel loose and/or start to separate.

As the gums begin to recede, the teeth may begin to feel loose because there is less gum to provide support. They also may appear as though they are separating because the teeth get narrower beneath the gums and when the gums recede, spaces between the teeth become apparent.

This is a sign of infection.

The Pennsylvania periodontists in DiGiallorenzo’s practice use the laser-assisted new attachment procedure- referred to as LANAP- to treat gum disease.

LANAP has so many benefits over traditional treatment, Pennsylvania periodontist Dr. DiGiallorenzo said.

LANAP offers treatment that doesn’t require cutting the gums and suturing them back in place after the scaling and root planing is complete. It also provides a quicker recovery time for patients because it less invasive and less painful. LANAP also eliminates bacteria.

Laser-assisted new attachment procedure- commonly referred to as LANAP- is gainly popularity in the treatment of periodontal disease because of the many benefits it offers over traditional periodontal treatments, which are more invasive.

PHILADELPHIA, PA – As a result of his use of the laser-assisted new attachment procedure, commonly referred to as LANAP, Philadelphia periodontist Dr. David DiGiallorenzo has identified what he considers to be the top five benefits of LANAP over traditional periodontal treatment.

DiGiallorenzo, who also is a Philadelphia dental implants specialist, has used LANAP in his practice since early 2009.

The benefits of LANAP that he has identified are as follows:

Less Invasive
LANAP was developed by Dr. Robert H. Gregg and offers an alternative to treating gum disease with traditional scalpel and suture surgery. After a patient has been diagnosed with gum disease, the LANAP is used to gently pass between the gum line and the teeth to vaporize disease and leave healthy tissue in its place. LANAP uses a fiberoptic that is equivalent in size to about three human hairs. The process uses ultrasonic root debridement to clean root surfaces, followed by a different laser setting to clot the blood so the periodontist gets a good seal, thus eliminating the need to use sutures.

“That means no cutting, no stitches and very minimal tissue removal to reduce pocket depth,” DiGiallorenzo said. “This is a bone-building procedure that doesn’t require removing much gum tissue.”

Quicker Recovery
Because of the less invasive nature of LANAP, recovery time is reduced and most patients who have the procedure performed at DiGiallorenzo’s Philadelphia periodontics practice require no time off work or other activities. Patients recovering from the procedure also experience less bleeding, fewer infections and quicker post-op time, he said.

Less Pain
A less invasive procedure also equates to less pain during treatment. Cutting into the gums requires administering a local anesthetic so that the doctor can peel back the gums and remove the plaque buildup from the teeth. The small laser makes it easier to get in between the gum and the teeth to accomplish the same thing, DiGiallorenzo said.

Eliminates Bacteria
LANAP removes the infection that coincides with gum disease.

Less Expensive
It seems there seldom is a time when a preferable procedure is cheaper than the traditional option, but that isn’t the case with LANAP, when patients consider the fact that they don’t have to spend as much time in the dental chair, said Gregg, the laser periodontal therapy developer.

Learn More
If you would like to learn more about LANAP or you would like to schedule a free consultation one of the Philadelphia periodontists in DiGiallorenzo’s practice to determine if you are a candidate for non-surgical treatment, please call (610) 409-6064 in Collegeville; (570) 322-4741 in Williamsport; or visit Dr. DiGiallorenzo’s Web site: http://www.perioimplants.us/.
